About a week ago, I made an offer to mail out a financial data base I 
created in Lotus Approach.  Several people wanted more info and / or a copy. 
 Let me explain more precisely what I have, and then I'd like anyone 
interested to reply using the "form" below, just so I won't forget anyone 
and so I do not need to have a bunch of followup threads with a bunch of 
individuals.

Approach is Lotus's data base product, somewhat comparable to Access.  It 
lets you create data input forms, reports, mailing labels, etc.  The data 
itself can be stored in many different formats, including .wk*, .xls, and 
dbf (Foxpro or dBase)  It's pretty inexpensive, and may be worth your while 
to buy if you have no system in place right now and just want to reuse my 
work.

I collected paper forms from all full and associate members of Cornerstone 
which were essentially mortgage prequalification forms.  I also collected 
some additional information.  I created an Approach DB which includes tables 
for households, individuals, and a table of financial data about different 
unit types.  I created a report which spits back the prequalification data, 
with space for requests for corrections / new data.  I also created summary 
reports which showed the group's ability to afford mortgages with no names 
attached.
