In LEED buldings, commissioning is a pre-requisite and must be built  
into the project from the beginning:

-Engage a commissioning agent
-Include commissioning requirements in the construction documents
-Write a commissioning plan and use it throughout design and  
construction
-Verify installation and functional performance of all system
-Document results and develop a commissioning report

This can cost up to $1.50 per square ft but most people say it saves
