We have now arranged our meeting minutes so that there is a sort of 
summary at the top, followed by the more detailed write-up of 
discussions, rationales, etc. Thus, decisions are easy to locate when 
scanning the minutes. 

Formerly, decisions appeared in the minutes in the format
*Agreed:........................."
Maybe they still do. 

It has been repeatedly suggested here that we have a "log" notebook of 
some sort that logs actual decisions. We haven't gotten to it, in part 
because so seldom would a few lines really do justice to the matter. 
Perhaps there could be a log of what was decided upon, with reference to 
the relevant minutes. 

Meeting minutes have been very valuable over our eleven years, especially 
for things that happened before some people were here.
