One rather minimal action which seems prudent is to
collect basic contact information for each householder
in the Community--their adult children's names,
addresses, phone numbers, emails; same for living
siblings or other relatives or friends important to
them, possibly their primary care providers info. So
in case of sudden change in health status the care
group is in a position to get family and friends
informed and involved. Also it would need to be
updated at least annually. The collection of this
info alone would no doubt spark a useful discussion.
