Hi Every One,
I am a resident of Sunward CoHo in Michigan.
We are preparing a survey for our community about our common meal  
program.

I'm betting other CoHo communities have surveyed their membership with  
regards to their common meals program.
It would be a great help to us in designing our survey if we could see  
some examples of surveys from other communities.


Is there anyone out there who has a common meal's program survey that  
they'd be willing to share with us?  We're specifically interested in  
the survey questions and answering rubric.
