Looks like I may be the first post of the year!  Happy New Year!

I am looking to create a community member self evaluation Form to use with
all of our members in January.  This is not necessarily for a sociocratic
or committee role (although a template of that would be very helpful), but
something more comprehensive.

Do any of your communities use self evaluations forms?  If so, would you be
willing to share yours?
