The facilitator for any specific meeting puts out a call for agenda items, 
which are sent to him or her by committee heads.  When submitting an agenda 
item, we are supposed to give the topic, the estimated time, the desired 
outcome (decision, discussion,etc) as well as if the issue is time-sensitive.
It is up to that facilitator to put together the agenda, which is sent out to 
everyone before the meeting.  If there is a proposal to be consensed upon, all 
of the relevant information is distributed via email well before the meeting, 
so that people come somewhat prepared.

During a meeting, sometimes some things take longer, etc.  The facilitator 
makes most of the judgement calls, sometimes putting them out to the group or 
to the committee responsible ("can this wait til next meeting, or do you really 
need a decision today?" etc.)
